Information you send us

The contact form asks for your name, email address, organization, reason for contacting us, and message. We use that information to review and respond to your inquiry. Organization is optional.

Please do not include patient names, medical details, or other health information in the form or in an email to our general contact address.

Website services

This website is hosted on Netlify. Netlify also processes contact-form submissions and provides spam filtering.

The site uses Google Fonts to display its typefaces. Your browser connects to Google to load those fonts.

Advertising and analytics

This website does not use advertising trackers or audience analytics. If that changes, we will update this notice.

Information used in patient programs

Information used to coordinate support in a patient program is handled through the arrangements established with the participating health plan or provider organization. This website’s contact form is for general inquiries, not patient-care communication.

You can ask a question about information you have sent us, or ask us to stop contacting you about a Cubie program, by emailing hi@cubiehealth.com.
