Who we are
What is Cubie Health?
Cubie Health is a care-support team that works with heart doctors and health plans. Our team includes registered nurses, dietitians, and social workers. We help coordinate support between appointments.
How do you work with my doctor?
In participating programs, we coordinate with your treating team and follow the agreed care plan. We bring questions and relevant updates back to the team. We do not change your treatment on our own.
Your doctor’s office can confirm whether it is participating in the program described in your invitation.
How did you get my information?
In participating programs, a health plan or provider organization shares information needed to arrange support. The source depends on the program. You can ask your doctor’s office or Cubie about the invitation you received.
We do not sell your personal information.
What support includes
The support available depends on your program and care plan. It may include:
- Check-ins between visits. We ask how you are doing and help bring questions or changes to the treating team.
- Medication follow-up. We help coordinate questions about following the plan your prescriber has given you.
- Support after a hospital stay. We help coordinate follow-up appointments and questions about the next steps.
- Help with everyday obstacles. We help identify practical issues, such as difficulty arranging an appointment or getting to one.
What does it cost me?
If you were invited through a participating Cubie program and are eligible, there is no additional fee or copay for Cubie support.
Your usual costs for medical visits, tests, and medications still apply.
Your invitation or the organization that invited you can help confirm whether you are eligible.
Your privacy and your choices
What will Cubie never ask me to do?
- Give us a credit card, bank details, or payment for Cubie support.
- Treat a Cubie recommendation as a prescription.
- Change treatment without the treating clinician’s direction.
Cubie does not diagnose conditions or prescribe treatment. If someone claiming to be from Cubie asks for payment or bank details, do not provide them. Contact your doctor’s office or email hi@cubiehealth.com to check the contact.
What if I do not want to take part?
You can decline. Tell the person who contacts you, or email hi@cubiehealth.com, and ask us to stop contacting you about the program.
Choosing not to take part does not change your access to care or your health-plan benefits.
Can I ask a question before deciding?
Yes. You can check with your doctor’s office or email hi@cubiehealth.com. Please do not include medical information in your email.

Do not email Cubie or wait for a call back. If you are worried about a health problem and it is not an emergency, call your doctor’s office.
